Immunization
According to the Oklahoma State Law, no minor child shall be admitted to any public, private, or parochial school unless such child can present to the school authorities certification that he/she received a test or vaccination for measles, rubella, polio, diphtheria, tetanus, and whooping cough or is likely to be immune as a result of the disease.
Verification that all tests or immunizations have been administered may come from a statement from the State Board of Health or a licensed physician. The child may be exempt from
immunization or test upon a statement from a licensed physician that such immunizations or test will endanger the life or health of the child; or upon a writen statement from the
parents or guardian objecting to such test or immunization for medical, religious, or personal reasons.
